Please note: No transactional forms should be sent to HRSS via email:

Employee Change Form (ECF) | Uploaded to DTS when an employee's details require amending in Electronic Staff Record (ESR), examples include, but are not limited to: - Additional payments - Future dated changes - Secondment amendments - Salary - Change of role that is not part of a recruitment |
Secondment Control Form (SECF) | Uploaded to DTS when an employee is seconded in or out of the organisation |
Disclosure and Barring Service (DBS) check Information Form | Uploaded to the DTS with a Job Description (JD) and proof of identity. |
Flexible Hours Form | Uploaded to the DTS |
Flexible Retirement Form | Uploaded to the DTS |
