Below is a table that confirms what forms should be sent to HR Shared Services (HRSS) through the Document Transfer Service (DTS).

Please note:  No transactional forms should be sent to HRSS via email:
 

Forms uploaded to the Document Transfer Service
Form Name When is it used?
Direct Hire Form

Uploaded to the DTS to add the following to ESR:

- Non-Executive Directors (NEDs)
- Chair, Vice Chair, Alternate Vice Chair
- Honorary
-Volunteer

Leavers Form

Uploaded to the DTS when an employee is leaving the organisation

Employee Change Form (ECF) Uploaded to DTS when an employee's details require amending in Electronic Staff Record (ESR), examples include, but are not limited to:

- Additional payments
- Future dated changes
- Secondment amendments
- Salary
- Change of role that is not part of a recruitment

Secondment Control Form (SECF) Uploaded to DTS when an employee is seconded in or out of the organisation
Disclosure and Barring Service (DBS) check Information Form Uploaded to the DTS with a Job Description (JD) and proof of identity. 
Flexible Hours Form Uploaded to the DTS
Flexible Retirement Form Uploaded to the DTS

Internal Recruitment Form (IRF) Created in Trac when an internal candidate is recruited to a role
Candidate Enrolment Form (CEF) Created in Trac when an external candidate is recruited to a role
